How to combine the image tag with radio button values

I'm designing a table where there's radio buttons in one column and in the next column are titles to a list of topics/error messages. My problem is, I want to be able to select a topic and then click on the "add" or "update" or "delete" button on the top of the page and then have it sent to the appropriate page corresponding to the button clicked and to be able to bring up the correct topic selected by the radio buttons. How do I do that?

I have tried and tried but just cannot get a good clear picture of this if you have something started that I could take a peek at let me know where I can see it.
